Despite an excellent safety record, Tai Shin Doh / Tai Shin Jitsu / Self-Defense is a contact activity and injury of any type is possible. By participating in these classes, you agree that you have determined that this activity is age appropriate for the student, and on your own behalf and on behalf of the participant assume all risk of injury. It is also understood that discussing or practicing scenarios involving violence and relying solely on class training may not prepare someone for encounters in real-life situations. By signing you waive, release, discharge, and agree to give up your right to any and all claims, liabilities, and causes of action against any and all affiliated people, organizations, and locations where this class is held including but not limited to Beachwood Academy LLC., Gymnastics & Parkour School, ClassPass Inc., Tai Shin Doh, Allen Roth, and any and all instructors of this class for you, your child, and your guest while participating in or while on the grounds of the program. Furthermore, you recognize that failure to keep financial obligations current will result in the student's loss of participation and testing privileges. Finally, at times you will receive class and schedule updates via email and/or text. You can unsubscribe from our email list at any time.
I also understand that cameras are in use on the premises. I hereby permanently release and waive compensation whatsoever, for the use of pictures and videos, I, my child, or my guest may appear in while on the premises and/or participating in this activity which may be used for training and/or advertising purposes.
I have read this waiver, and I fully understand its' contents. I am signing voluntarily and intend for it to be a complete and unconditional release of liability to the greatest extent allowed by law.
